Turbo Shaft Failure

Making a couple of runs my turbo died out of boost and started making a whirling sound. I drove home ~1 mile like that. Took off my intake and the pictures below are what I found. The nut and portion of the shaft, along with some pieces were in the intake filter. Taking a look at the shaft it looks like it had a previous crack in it from the rust. Axial play on the turbo is still good and there is no oil in the compressor. Not sure whether a turbo rebuilder would be able to use my T04S housing or not. I'm hoping so as its got my V- band on it. So I guess I need a rebuild any suggestions? Would it be better to buy a brand new turbo? Can someone machine out the housing for a T04R wheel? Let me know what you think.

Wow, looks straight-up like a manufacturer defect on the shaft.
Yikes.

I don't think it has anything to do with your EGT's.

Turbo was new about 500 miles on it total. Turbine actually looks fine to bad the shaft is probably toast...
