i'd have voted on the TB myself, but wanted some outside views. when pressing the gas pedal, there's a little play for about an inch, then you can feel it grab and start moving the throttle cams. the cams themselves on the backside of the TB have a little slop in them and the cables aren't tight. the cam on the front side appears to be in it's rest position. even if they were just partially open, i wouldn't think it would cause it to rev that quickly. either way, i'll try to get the elbow off before the TB tomorrow and see if the valves are indeed stuck open a bit.

Try and see if you can spot evidence that the UIM was removed - UIM nuts / bolts show signs of removal, throttle cable removed, etc.
If the UIM was removed, almost guarantee the throttle cable is binding.
This happened to be when I rebuild my motor - pegged the tach and scared the **** out of me!
The actuator cams on the throttle body itself cannot prop the throttle plates open that much - 4kRPM at the max.
If the throttle body looks kosher (nothing jamming it externally), I would remove the throttle cable...shake it a few times and push the gas pedal several times to remove the twist in it...and then reinstall.
If this doesn't fix it, the throttle plates are sticking...which means R&R of the throttle body, which is going to take some time.

sorry, need to update this. i was in the process of taking everything apart finally the other day. i noticed there was a screw missing from the top of the t-body. i glanced down and saw some threads sticking up around one of the throttle cams. turn out, the screw vibrated out and wedged itself in there. it wasn't much, but it was enough to cause some damn problems. go figure, huh?
