Idle Problem... in depth. Removing

Hey folks, just a quick question about the idle, i know its been hacked to death, but i think i have a bit different problem... When i start up the car in the morning, she kinda doesn't want to stay started, if i assist a bit for about 20 seconds or so, it will stay on... But after i rev it, the idle will stay HIGH around 1100 rom and misses alot too... But if i put my foot on the brake and disengage the clutch a bit and put the motor under load it will return to an almost perfect idle... any ideas? The engine is failry tight and there are no vacumn leaks

Edit: What can i remove from my engine to make all these problems go away... What do most people remove? I dont have any cats on the car, air pump is removed....

When its cold, it doesn't want to idle very well until it gets hot then it idles great! Then when i give it some rev's it stays up at like 1100 and is very rough, until i give it some load (brake on let clutch out a bit) it will resume its low normal idle...
