Still running all your pollution control? If so, try unplugging the colored solenoid plugs, one by one, to see if any of them affect your idle. Sometimes the air control valve, etc, leak air by, and screw up the idle.

I'll try the solenoids tomorrow. I can't wait until I tear down the intake and fix my 5/6 port sleeves....hopefully I'll be ridding myself of all the emissions crap and replacing a lot of exhaust gaskets so the thing doesn't reek anymore....

I turned the screw all the way in and it idled fine, just have to give it some gas sometimes when cold to keep it from dieing. Anyone else think that might be the OP's problem?
It seems likely. It would almost have to be one of the three: AWS, BAC, or fast idle cam/thermowax. But the AWS should put the idle up to 2500-3000, and then only for 17 sec, so that does not seem consistent with the symptoms. Since the BAC is active when warm, and the fast idle cam is not, the fact that it smoothes out when warm points to the fast idle cam/thermowax.

Just to update...definitely was a TPS issue. I don't have any LED's around and at the time I didn't have a voltmeter so I did some blind tuning. Pulled the screw out two full turns and now she idles closer to 1500rpm cold, 800rpm hot, no fluctuations in or out of gear. Need to get the meter out, though, and set it proper.

Now I just need to sort out the popcorn idle...
