One of my GSLs made fan sounds a little like standing behind a jet liner at an airport, even at idle. Turned out to be a combination of the fan clutch being stuck and the fan shround not being properly mounted. A new fan clutch should fix the problem and check to be sure the shrouds that control the air flow are mounted correctly.

With enough air through the radiator, i.e., speed, the fan free wheels. On the other hand, if you are driving on a warm day in stop and go city driving, the fan clutch is locked and and doing it's job.
Not hearing the fan at 60 mph in 5th gear may be because the other noises are louder than the fan and you can't hear it.
I have the same problem with my 2000 van, The fan is very noisy when first starting up and driving. Once the temps begin to come up, it free weels and I never notice it. I think this is built into the vehicle, but don't know why. It stops being noisy after the first mile, unless it is a very cold day.
An easy test for the fan clutch is: When the engine is off, it should have some resistance to turning by hand, not a lot. With the engine running and the fan turning, sticking a broom, carefully on the blades, the fan shoud stop. Use the working end of the broom, not the handle.

Being the adventurous type that I am, I use my finger for Trochoid's test, but you do need to know what you're doing. Also, I practiced with some cardboard the first few times. And, if the fan clutch is seized, you don't want to be doing this. Disclaimer: Don't try this at home.

Off topic: After all these years of being a member here (more than 5 years total), I finally hit 700 posts!
I did this once and I nearly lost my finger! I would definitely take trochoid's advice on this one.
